Capture the **volunteer's contact, relationship to the school** (parent of a student, community member), the **shifts they can take**, and useful **notes** (what they can bring, t-shirt size, who they're volunteering with). Two things schools specifically need: a **background-check status** field — most schools require a check (and sometimes training) for volunteers, especially anyone with unsupervised student access — and a **minor toggle**, since a student volunteer needs parent/guardian permission, which switches the signature to the guardian. Round it out with an **emergency contact** and a short **acknowledgment** to follow the school's volunteer and child-safety policies. Keep it simple and friendly to fill out, collect only the information you need, and store it securely — volunteer and family data deserves the same care as student records.
