How does "before" placement work?

For each listed page N, the copies are inserted immediately before page N. So "duplicate page 3, 2 copies, before" gives you [1,2, copy3, copy3, 3, 4, ...].

Will bookmarks pointing to old page numbers still work?

Bookmarks are preserved by pdf-lib but their target page indices may shift if pages are inserted before them. Run Auto-Bookmark from Headings on the result if needed.
