Two cover modes: "auto-generate" (creates a simple title page with the filename, page count, and optional title prefix) or "first uploaded PDF as cover for the rest" (use a designed cover from your designer for every batch). In "first PDF" mode the cover preserves its source size; in "auto" mode the cover is US Letter (612 x 792 pt). The document's pages are unchanged either way.
