How does the Fillable PDF Form Builder work?
Generate a blank fillable PDF from a simple list of "key=label" lines — each becomes an AcroForm text field that anyone can fill later in any PDF reader (Adobe, Preview, Foxit) or via our Fill PDF Form / Pre-populate from CSV tools. The "key" is what shows up in form-extract output and CSV pre-populate headers; the "label" is what the user sees on the page. Useful for quickly authoring a survey, intake form, sign-up sheet, or evaluation rubric without leaving the browser. Each field is a plain text box; checkbox / dropdown authoring is on the roadmap.
