undersigned RELEASES AND HOLDS HARMLESS the studio, its owners, instructors,
and staff from claims arising from ORDINARY NEGLIGENCE in connection with
classes, rehearsals, and recitals. This release does NOT apply to gross
negligence or willful misconduct. Consent is given for emergency medical
care if the student is injured and a parent cannot be reached.

Three legal points shape the form. First, **minors**: a student under 18 generally cannot be bound, so a parent or guardian signs and accepts the terms on the child's behalf — and even then, a parent's pre-injury waiver of the *child's own* claims is limited or void in several states, which is why the release is written to apply "to the fullest extent permitted by law" rather than overpromising. Second, the **waiver** is enforceable for ordinary negligence in most states but never for gross negligence or willful misconduct, and it does not replace the studio carrying liability insurance. Third, the **photo/recital release** should be explicit and, for children, consented to by the parent — studios routinely photograph classes and recitals for promotion, and using a child's image without consent is a common complaint. Keep the form to one page, state tuition and fees plainly, and confirm the waiver language against your state's rules before the season starts.
