INCIDENT NARRATIVE

I was walking through Hyde Park at approximately 4:45 PM on 2026-04-22. I encountered the animal owner with the dog (Rex) on a non-retractable leash. The dog was approximately 6 feet from us, on the leash but lunging and barking aggressively.
The dog suddenly broke from the leash hold (the owner appeared to lose grip) and ran toward me. The dog bit my right calf and ankle before the owner regained control.
The owner appeared genuinely apologetic. I immediately took photos of the dog and exchanged information. The owner appeared cooperative.
No other people present at the immediate moment of the incident; one witness (Casey Brooks, +1 503 555 0203) was 50 feet away and saw the incident from start to finish.

About this template

An animal-bite incident report serves multiple purposes: medical record, insurance claim documentation, public-health (rabies surveillance), legal-liability documentation, and animal-control reporting. State and local public-health authorities require reporting of animal bites within state-specific timeframes (typically immediately or within 24 hours). Failure to report can be a misdemeanor in some jurisdictions. The report should document: (1) Animal description (species, breed, size, colour, vaccination status if known); (2) Animal owner identification; (3) Incident location and circumstances; (4) Bite victim details (name, age, bite location); (5) Witness information; (6) Medical attention sought (rabies risk evaluation, antibiotic prophylaxis, tetanus update); (7) Authority notification (Animal Services, public health). Dog-bite liability is generally STRICT LIABILITY for the owner under most state laws - the owner is liable for damages regardless of fault or knowledge of vicious propensity. Some states (the "one-bite rule" states like New York, Texas, and Virginia) require the bite victim to prove the owner knew of the dog's vicious propensity for liability to attach; most states have moved beyond this. State-specific dog-bite laws: California Civil Code ยง3342 (strict liability); Oregon ORS 31.360 (modified strict liability); Texas Civil Practice and Remedies Code Chapter 84 (one-bite rule); New York McKinney's NY Agriculture and Markets Law ยง123 (some elements of one-bite). Insurance coverage: most homeowner/renter insurance covers dog-bite liability up to policy limits ($100K-$500K typical); some breeds are excluded by some insurers (Pit Bulls, Rottweilers, German Shepherds, etc.). Personal-injury attorneys handle dog-bite cases on contingency given fee-shifting in some states; substantial cases (broken bones, scarring, psychological trauma, attack on minor) typically warrant attorney representation.

When to use it

  • After being bitten by an animal.
  • Witnessing an animal-bite incident.
  • After a child is bitten (parent reporting).
  • Animal owner documenting their animal's bite of another.
  • Filing insurance claim for dog-bite injury.

What to include

  • Reporter identification.
  • Specific date, time, location.
  • Animal description with owner ID.
  • Bite victim details.
  • Detailed incident narrative.
  • Witness information.
  • Medical attention received.
  • Reports filed with authorities.

Frequently asked

YES, immediately. Most jurisdictions require reporting bites; failure to report can be a misdemeanor. Animal Services can verify the animal's rabies vaccination status, order quarantine for observation, and create the official record needed for liability claims and future enforcement against the owner. Reporting is often online or via phone.
โš  Legal disclaimer. Animal-bite reporting is required by most jurisdictions; failure to report can be a misdemeanor. Dog-bite liability is heavily state-specific - California Civil Code ยง3342, Oregon ORS 31.360, Texas Chapter 84, New York Agriculture and Markets Law ยง123, and others vary in their strict-liability vs one-bite-rule frameworks. For substantial injuries, consult personal-injury attorney - many work on contingency given state fee-shifting. Public-health reporting (rabies surveillance) is separate from civil claims; both are typically required.
