Can I have nested bookmarks (a tree)?

v1 supports a flat list. For chapter > section nesting, the next release will add an indent syntax. For now, prefix titles with " - " to imply hierarchy visually.

Where can I get the list of headings?

If your PDF has a clear typographic structure, use Generate Table of Contents to detect headings automatically and add bookmarks in one step.
