1. MEMBERSHIP
   Package: Monthly Massage or Facial     Start date: June 1, 2026
   Included: One 60-minute massage or classic facial per month, 10% off additional services and retail, free aromatherapy upgrade.
   Rollover: Unused monthly service rolls over for up to 3 months while membership is active; rollover credits are forfeited at cancellation.

3. AUTOMATIC RENEWAL
   This membership AUTOMATICALLY RENEWS month-to-month after any minimum term until the Member cancels per the cancellation policy. The Member may cancel an automatic renewal at any time as provided here and under applicable automatic-renewal law.

4. CANCELLATION
   Membership: Cancel anytime with 30 days' written notice after a 3-month minimum. No cancellation fee after the minimum term.
   Appointments: Appointments cancelled with less than 24 hours' notice, or no-shows, forfeit that month's service.

5. STATUTORY COOLING-OFF / RIGHT TO CANCEL
   The Member may CANCEL this agreement without penalty within 3 business
   day(s) of signing (or longer if required by the Member's state health-studio
   law) by delivering written notice to the Spa at the address above. Refunds of
   prepaid amounts will be made as required by applicable law.

6. SERVICE TERMS & ACKNOWLEDGMENT
   Spa services (massage, facials, body treatments) are for relaxation and
   wellness and are NOT medical treatment or a substitute for medical care. The
   Member will disclose relevant health conditions, allergies, pregnancy, and
   medications before each service, and understands the provider may decline or
   modify a service for safety. The Member uses the facility and services at
   their own risk and agrees to follow spa policies.

7. GOVERNING LAW
   Governed by the laws of the State of Illinois, including its
   health-studio and automatic-renewal statutes.
